A British-Nigerian influencer died after a penis-enlargement injection led to a fatal pulmonary embolism, according to newly published accounts of a London inquest.

Igho Ubiribo, known to followers as Tiny or Ego, was 43.

He died in Bangkok on March 6. The details of the procedure and the findings about his death are emerging in reports published this weekend.

According to TMZ, Ubiribo was vacationing in Thailand with his wife, Danielle Simba Allen, when he underwent the cosmetic procedure at a clinic. Coroner Jean Harkin attributed his death to a pulmonary embolism caused by that procedure.

He reportedly received injections containing hyaluronic acid and the anesthetic lidocaine. Afterward, he had a massage, developed chest pain and lost consciousness.

He was taken to Sukhumvit Hospital, where he was initially able to answer questions despite the earlier episodes of unconsciousness. His wife told medical staff about the filler treatment he had received at the clinic.

But he lost consciousness again before a planned CT scan. Doctors transferred him to intensive care and tried to resuscitate him for roughly 100 minutes.

He could not be saved.

The postmortem findings were especially disturbing: Metro reports that material found in Ubiribo’s lungs matched the hyaluronic acid used in the injection. Pathologist John du Parcq said the findings were consistent with a pulmonary embolism and agreed with the Thai autopsy.

The outlet reports that the treatment involved 40 milliliters of hyaluronic acid and lidocaine. It places the procedure during the couple’s March holiday, when they were staying at a Marriott hotel in Bangkok.

Ubiribo had around 185,000 Instagram followers. His funeral was held in Kensington, London, where his childhood friend, Nigerian singer Davido, paid tribute to him.

Davido remembered his friend’s energy and presence and spoke about the weight of his absence. Behind the shocking circumstances was a family grieving a husband and a circle of friends grieving someone they had known for years.

The Food and Drug Administration warns that accidentally injecting dermal filler into a blood vessel can cause serious harm. A filler can block blood flow, with possible consequences including tissue death, blindness or a stroke.

The agency says dermal-filler injections should be treated as medical procedures. It recommends a licensed health care provider who understands anatomy and has training and experience with the procedure, including recognizing complications and managing them when they occur.

Hyaluronic acid is among the materials used in FDA-approved fillers, but approval applies to particular products and uses. The agency’s approved uses include specified treatments involving the face and hands.

The word “filler” can make a procedure sound small. The warnings attached to these products are anything but.

In Ubiribo’s case, the newly reported inquest findings connect the fatal embolism to the cosmetic injection. A holiday with his wife ended with his family bringing him home for a funeral.
