Sean “Diddy” Combs was taken out of federal prison in an ambulance after a late-night medical emergency, according to a new report.
The music mogul reportedly left FCI Fort Dix in New Jersey around 1 a.m. Saturday for treatment of a head injury.
He was back behind the prison gates before sunrise.

TMZ reports that an ambulance carried Combs off the prison grounds early Saturday morning so he could receive care for the injury. The outlet has not confirmed which medical facility treated him, and the Bureau of Prisons has not publicly explained what happened.
Sources described Combs as conscious and speaking when he was returned to Fort Dix before dawn. He was back inside within hours.
What caused the head injury remains unclear.
The short timeline is striking: the ambulance reportedly left after midnight, Combs received outside treatment, and prison staff had him back at Fort Dix before the morning began. No diagnosis, incident report, transport destination, or official prison account of the night has been released publicly.
That leaves the central question unanswered: How did one of the most closely watched inmates in America end up with a head injury serious enough to require an ambulance trip outside the prison?

The reported emergency comes during a turbulent stretch of Combs’ time at Fort Dix.
NBC News reported earlier this month that Combs had been moved from dormitory-style housing into the special housing unit after allegations surfaced about preferential treatment and contraband inside the prison. His representative pushed back on the attention surrounding his incarceration and said Combs was serving his sentence while awaiting a decision on his appeal.
Now there is a fresh incident—and, so far, very little official information.
The move into special housing had already put Combs under a brighter spotlight inside Fort Dix. A late-night ambulance transport adds another serious question to a period that was already drawing scrutiny.
There has been no public statement identifying the cause of the injury, no confirmed account of where Combs received treatment, and no indication that the ambulance transport changed his custody status. What has been reported is that he left the grounds for treatment and returned within hours.

Combs is serving a 50-month federal sentence after a jury convicted him on two counts of transportation to engage in prostitution.
The Associated Press reported that U.S. District Judge Arun Subramanian imposed the sentence in October 2025, along with a $500,000 fine and five years of supervised release. The jury acquitted Combs of the more serious racketeering-conspiracy and sex-trafficking charges, and his lawyers said they would appeal.
He was later transferred to Fort Dix, a low-security federal facility in New Jersey.
A medical transport at 1 a.m. is not ordinary prison movement. It means staff decided the situation required care beyond whatever could be handled inside the facility at that moment.
The fact that Combs reportedly returned before sunrise and was conscious is reassuring. But it does not answer how the injury happened—or whether prison officials are investigating the circumstances around it.
For now, those are the details still being kept behind the walls at Fort Dix.
